How Verification Works
Email a clear photo of your insurance card, along with your phone number and date of birth, to FiveElementCare@gmail.com.
Our team promptly verifies your coverage and contacts you with the details. Please allow us a little time to complete the check.
Once your coverage is confirmed, our friendly staff reaches out to schedule an appointment at your convenience.

Self-pay
If you don't have insurance — or your plan doesn't cover your treatment — you're always welcome as a self-pay patient. Our self-pay packages lower your per-visit rate, so consistent care stays within reach. Same-day appointments are often available too — just call our office, or book online.
